چالش‌های برنامه پاسخ نظام سلامت جمهوری اسلامی ایران به بحران و راهکارهای برون رفت (مطالعه موردی: مرکز بهداشت استان قم)

چکیده .
کشور ایران به دلیل موقعیت جغرافیایی و شرایط فرهنگی، سیاسی، اجتماعی و ...، همواره مستعد وقوع انواع بحران‌ها بوده است. بدین منظور، وزارت بهداشت برنامه‌ای تحت عنوان «پاسخ نظام سلامت در بلایا» جهت مدیریت بحران در نظام سلامت تدوین و به دانشگاه‌های علوم پزشکی ابلاغ کرده است. با توجه به سابقه اجرا این برنامه در مرکز بهداشت استان قم به نظر می‌رسد نتوانسته جوابگوی کامل نیازهای مدیریت بحران و به تبع آن پدافند غیر عامل در این حوزه باشد. هدف این پژوهش، شناسایی چالش‌های اجرای این برنامه در مرکز بهداشت استان قم و ارائه راهکارهایی جهت برون‌رفت از این شرایط است. این پژوهش از روش کیفی توصیفی-تحلیلی استفاده و با استناد به ادبیات پژوهش، اطلاعات سازمانی، و مصاحبه‌های تخصصی، چالش‌های برنامه را بر اساس ۱۲ معیار مدل بورک-لیتوین استخراج کرده است. 45 چالش مهم بر مبنای مقوله‌بندی، با 4 مقوله اصلی و 12 مقوله فرعی استخراج شده است. با ارائه راهبردها و راهکارهای برون رفت انتظار می‌رود نتایجی شامل: اجرا‌، ارتقاء‌، اثربخشی‌، کارایی و ‌بهره‌وری برنامه‌، پاسخ به بحران، ثبت اطلاعات صحیح، عملکرد کارکنان، رفتار جامعه، مدیریت بحران، مشارکت عمومی و نظارت بر برنامه حاصل شود. دستاوردهای حاصل از اجرای این پژوهش در حوزه مدیریت بحران و پدافند غیر عامل بهبود مؤثر نظام مراقبت‌های بهداشتی، ارتقاء اجزای نظام مراقبت‌های بهداشتی نسبت به بلایا و کاهش مخاطرات و عوارض بلایا در سطح جامعه عنوان شده است. در نهایت نتیجه انجام این مراحل به منظور جمع بندی الگو سازی شده است.

Due to its geographical location and cultural, political, social, etc. conditions, Iran has always been prone to various crises. For this purpose, the Ministry of Health has developed a program entitled "Health System Response to Disasters" for crisis management in the health system and communicated it to the universities of medical sciences. Considering the history of implementation of this program in the Qom Provincial Health Center, it seems that it has not been able to fully meet the needs of crisis management and, consequently, passive defense in this area. The aim of this research is to identify the challenges of implementing this program in the Qom Provincial Health Center and provide solutions to overcome these conditions. This research uses a qualitative descriptive-analytical method and, citing research literature, organizational information, and expert interviews, has extracted the program's challenges based on 12 criteria of the Burke-Litwin model. 45 important challenges have been extracted based on categorization, with 4 main categories and 12 subcategories. By providing exit strategies and solutions, it is expected that the following results will be achieved: implementation, promotion, effectiveness, efficiency and productivity of the program, response to the crisis, recording accurate information, staff performance, community behavior, crisis management, public participation, and program monitoring. The achievements of this research in the field of crisis management and passive defense are effective improvement of the health care system, promotion of the components of the health care system with respect to disasters, and reduction of the risks and consequences of disasters at the community level. Finally, the results of these steps have been summarized in a model.
